1. Apply by completing and submitting the Member Application on this website. You can apply directly online or by downloading, printing and submitting a written application.
You have a choice of two dates to specify as your start date. One is the date you submit your Member Application online or the postmark date on the envelope in which you mail your application. The second option is for you to choose a future date when you want your participation to begin. Either choice marks your start date. Your start date will affect your first financial gift date.
2. Expect your member welcome pack and CHM membership card within two weeks of submitting your application.
3. Send us the forms indicated in your member welcome pack.
